Form: Application for AA/AS Degree
Purpose: Apply for an Associates of Arts and/or Associates of Science from MSJC. You may need to submit an Application for AA/AS Degree when applying for multiple degrees from MSJC.

Form: Application for Certificate
Purpose: Apply for certificate, or for general education lower-division certification for transfer to a university from MSJC. You may need to submit an Application for Certificate when applying for multiple awards from MSJC.

Form: Duplicate Diploma/Certificate Request
Purpose: Request a copy of your diploma or certificate award

Form: Enrollment Verification Request
Purpose: Request an official enrollment verification, including proof of enrollment for employers, insurance providers, government agencies, or other third parties. Students may also request a Letter of Non-Attendance/Non-Enrollment through this form.
Enrollment verification requests submitted directly to Mt. San Jacinto College using the Enrollment Verification Request Form or for third-party forms completed by Enrollment Services are provided at no cost to the student.
Students needing an immediate enrollment verification may obtain one through the National Student Clearinghouse for a small fee. Visit the Enrollment Verifications webpage for additional options and ordering information.

Form: Foster and/or Homeless Youth Declaration
Purpose: Students who are under the age of 25 and are currently foster youth, or who have been foster youth in the past should use this form to obtain confirmation of their status as a foster youth. Submit the completed form and any supporting documentation to the EOPS/Guiding Light Department.

Form: Loss of Priority Registration and CA College Promise Grant and/or Pause Appeal
The form will be available only during the submission window. Please refer to the Important Dates for the term to view the exact date.
Purpose: 1. Students who have lost their priority registration due to exceeding 100 cumulative units of post-secondary credit. 2. Students who have been on two consecutive semesters of academic notice, progress probation, or a combination of the two, will lose their CCPG fee waiver eligibility. Submit this form and supporting documentation to appeal the loss of Priority Registration or the CCPG fee waiver based on an extenuating circumstance or significant academic improvement.

Form: Petition For Excused Withdrawal (EW)
Purpose: Request for a grade(s) of a W, NP, D or F to be changed to an EW due to illness, accident, or other unforeseen extenuating circumstance.

Purpose: Students who wish to enroll in more than the maximum number of units allowed for a term must meet with an MSJC counselor to discuss their academic plan and determine eligibility for an overload. If approved, the counselor will process the overload request during the counseling appointment. Overload requests are no longer submitted directly to Enrollment Services.
How to Request: Meet with an MSJC counselor in person or virtually to review your request. Counseling Department at MSJC | Mt. San Jacinto College.
Processing Timeframe: Same day, during the counseling appointment.
Form: Petition to Repeat
Purpose: 1. Petition to Repeat - demonstrate the necessity to retake a course more than the allowable 3 attempts or to retake a course that has a passing grade but needs to be retaken due to recency. 2. Work in Progress - Students who would like to retake a course they are currently enrolled in but are going to earn a substandard grade.

Purpose: Must meet with an MSJC Counselor to clear a prerequisite or to complete the necessary form to challenge a prerequisite or co-requisite of a course.

Additional Documentation Required: Transcripts, a statement and supporting documentation (course outline, syllabus, etc.)

Form: Registration Change Form
Purpose: 1. Request to be reinstated or added late to a course after Add Authorizations have expired (refer to the Important Dates for the term). 2. Request to change from one section to another of the same course, within the same term or if requesting to change to a lower-level course of the same subject (ie: Math-110 to Math-105). Requesting to change to a higher-level course of the same subject, requires meeting with an MSJC Counselor for a Prerequisite Clearance.

Form: Request for Credit for Prior Learning
Purpose: To demonstrate proficiency in a course eligible by requesting Credit for Prior Learning. You may be able to receive college credit through the approved alternative method (credit-by-exam, portfolio, workforce training certificates, industry-recognized credentials, etc.).

Form: Request for Evaluation
Purpose: Request to have previous coursework from another college or university evaluated. This form is also used for course substitution, academic renewal requests, AP, CLEP or IB scores, and to clear a language other than English (IGETC – Area 6) requirement with high school coursework equivalent to two years and a grade of “C-“ or better.

Form: Student Petition
Purpose: To petition situations that may be considered special circumstances which may warrant further consideration for adjustment or deviation from established procedures and policies of MSJC or to appeal a decision already made.

Purpose: Request updates or corrections to your student record, including your name, Social Security Number, date of birth, gender, email address, phone number or degree earned correction.
How to Request: Please visit the Welcome Center in person at any Mt. San Jacinto College campus location during normal business hours. The required forms are completed and processed in person and are not available for online submission.
Additional Documentation Required: Students must present valid photo ID and original supporting documentation. Copies, photographs, screenshots, or scanned documents will not be accepted.
- Social Security Number (Add or Correction) – Original Social Security card required.
- Name Change – Driver's license, marriage certificate, birth certificate, court order, divorce decree, or other legal documentation supporting the name change.
- Name Correction – Driver's license, birth certificate, marriage certificate, or other legal documentation.
- Date of Birth Correction – Driver's license, birth certificate, or other legal documentation.
- Gender Change or Correction – Birth certificate, court order, or other legal documentation.
- Email Address or Phone Number Change/Correction – Students should indicate if the request is needed for student account authentication or account access purposes.
Processing Timeframe: Same-day processing during your visit, when all required documentation is provided.
Important: Students must appear in person to submit these requests. Original documentation is required and will be reviewed at the time of service.
Form: Supplemental Residency Questionnaire
Purpose: To establish California residency.

Purpose: Request an official transcript from MSJC. Official transcripts are ordered online through Parchment and may be sent electronically or by mail. Students can also track the status of their order through their Parchment account.
How to Request: Visit the MSJC Transcript webpage to place an order through Parchment.
